I/we hereby give my/our permission for Faith Bible Church and any ministry of Faith Bible Church to use my/our image(s) and/or the images/videos of the minor children identified above, as well as name and geographical information, in media and publications related to Faith Bible Church.

I/we understand that the images may appear and be identified in various media outlets such as print, social media, television, website, online, newspaper, and radio in any Faith Bible Church publication or promotions.

I/we understand that participation in activities conducted by Faith Bible Church carry with them a degree of risk. I/we consent to participation in these activities.

I/we hereby also agree to indemnify and save and hold harmless Faith Bible Church, its officers, directors, volunteers, and employees from any loss, liability, damage or cost incurred due to my/our or the child(ren)'s participation in, or use of, Faith Bible Church facilities to the fullest extent permitted by law.
